目的应用无创血流动力学监测仪观察单硝酸异山梨酯治疗冠心病心力衰竭的临床疗效。方法按照美国纽约心脏病协会的心功能分级标准,选取本院2007年10月至2008年10月收治于心血管病科的冠心病心力衰竭患者共120例,按NYHA分级分为I、Ⅱ、Ⅲ、Ⅳ共4组,每组再分设治疗组和对照组,对各组患者应用Bioz.com无创血流动力学监测得到11种血流动力学参数,进行统计学分析。结果用药后血流动力学监测仪监测各组患者的心输出量(CO)、心排血指数(CI)、左室每搏作功指数(LVSWI)、左心作功指数(LCWI)、每搏输出量(SV)、每搏输出指数(SI)、速率指数(VI)、外周阻力(SVR)、外周阻力指数(SVRI)、胸部液体含量(TFC)、收缩时间比率(STR)等参数较之用药前差异均有统计学意义(P<0.05)。结论单硝酸异山梨酯能显著改善冠心病心衰患者的心功能状况,提高生活质量;无创血流动力学监测到的血流动力学参数可作为评估冠心病心力衰竭的重要量化指标。
Objective To observe the clinical efficacy of isosorbide mononitrate in the treatment of heart failure with coronary heart disease by noninvasive hemodynamic monitor. Methods A total of 120 CHD patients with coronary heart disease admitted to our department of cardiovascular diseases from October 2007 to October 2008 were selected according to the New York Heart Association grading criteria. According to the classification of NYHA, I, II, Ⅲ and Ⅳ were divided into 4 groups. Each group was further divided into treatment group and control group, and 11 kinds of hemodynamic parameters were detected by Bioz.com noninvasive hemodynamics for statistical analysis. Results The hemodynamics monitor used to monitor cardiac output (CO), cardiac output index (CI), left ventricular stroke power index (LVSWI) and left ventricular cardiac function index (LCWI) (SV), stroke output index (SI), rate index (VI), peripheral resistance (SVR), peripheral resistance index (SVRI), chest fluid content (TFC), systolic time ratio Before treatment, the differences were statistically significant (P <0.05). Conclusion Isosorbide mononitrate can significantly improve the cardiac function and improve the quality of life in patients with coronary heart disease. The hemodynamic parameters measured by noninvasive hemodynamics can be used as an important quantitative index for the assessment of heart failure in patients with coronary heart disease.
