目的:探讨妊娠合并系统性红斑狼疮的临床诊断和治疗措施,总结临床诊治经验以改善妊娠结局。方法:回顾性分析2008年3月～2012年12月我科收入的35例妊娠合并系统性红斑狼疮患者的资料。结果:本组45例患者中并发狼疮肾炎9例,于痫前期6例,肾功能不全5例,关节炎3例,心包炎1例,因并发疾病严重而引产者2例,无孕妇死亡;胎儿宫內生长受限19例,早产12例,流产及死胎4例,新生儿死亡2例。结论:妊娠合并系统性红斑狼疮病情严重,应警惕并发症,及时诊断与积极治疗可降低母婴死亡率。
Objective: To investigate the clinical diagnosis and treatment of pregnancy complicated with systemic lupus erythematosus, summarize the experience of clinical diagnosis and treatment to improve the pregnancy outcome. Methods: The data of 35 pregnant women with SLE from March 2008 to December 2012 were retrospectively analyzed. Results: There were 9 cases of lupus nephritis in the 45 patients. There were 6 cases of preeclampsia, 5 cases of renal insufficiency, 3 cases of arthritis and 1 case of pericarditis. There were 2 cases of induced abortion due to complicated diseases and no pregnant women died. Fetal growth restriction in 19 cases, 12 cases of premature birth, abortion and stillbirth in 4 cases, neonatal death in 2 cases. Conclusion: Pregnancy complicated with systemic lupus erythematosus is seriously ill and should be alert to complications. Immediate diagnosis and active treatment can reduce maternal and infant mortality.
