目的探讨双额叶脑挫裂伤的诊断与治疗。方法回顾性分析32例双侧额底脑挫裂伤患者的临床资料。结果 32例患者行冠状开颅加去骨瓣减压术8例,冠状开颅骨瓣复位13例,冠状双额去大骨瓣术11例。术后随访6个月,根据格拉斯哥预后评分(GOS):恢复良好12例,中度残疾10例,重度残疾3例,植物生存2例,死亡5例。结论双额叶脑挫裂伤病情变化较快,动态头颅CT复查及正确掌握手术指征,早期积极手术治疗可有效降低患者的死残率。 Objective To investigate the diagnosis and treatment of double frontal lobe contusion and laceration. Methods The clinical data of 32 patients with bilateral contusion and laceration of the brain were retrospectively analyzed. Results 32 patients underwent coronary craniotomy and decompressive craniectomy in 8 cases, coronal craniotomy flap reduction in 13 cases, coronal double trepanation in 11 cases. Postoperative follow-up was 6 months. According to the Glasgow Outcome Rating Scale (GOS), 12 were well-recovered, 10 were moderately disability, 3 were severe disability, 2 were plant-based and 5 were fatal. Conclusions Double frontal lobe contusion and laceration changes rapidly, dynamic head CT review and the correct surgical indications, early active surgical treatment can effectively reduce the mortality rate of patients.
