山莨菪碱是从茄科植物唐古特莨菪中分离出的一种生物碱,其人工合成物代号654-2,为M胆碱能受体阻滞剂。近年来该药物的临床应用范围不断拓宽,现就国内文献报道在内科急、重病症中的应用作一综述。 1 呼吸系统 1.1 哮喘持续状态 卢凯祥用654-2治疗哮喘持续状态11例,结果10例于30分钟,1例于1小时后症状缓解。方法:654-210～15mg静注或从莫非氏管滴入。654-2对大脑皮层有镇静及呼吸中枢兴奋作用,具有抑制患者过度紧张和改善呼吸功能双重效应,有利于病情缓解。 1.2 急性肺水肿 应用654-2治疗急性心源性肺水肿24例和非心源性肺水肿7例均显效。其中9例是用吗啡、强心剂等治疗2小时无效后改用此药。1例高原性肺水肿经激素、氨茶碱、速尿等治疗2天无效,改用654-25mg静脉注射,间断用药2天后症状缓解,12天后痊愈。
