目的:了解国内文献关于降糖药药物经济学研究情况,寻求成本效果较好的用药方案,探讨目前国内降糖药成本效果研究存在的不足。方法:计算机检索中国生物医学文献数据库(SinoMed)、中国学术期刊全文数据库(CNKI)、万方数据库、维普中文科技期刊数据库(VIP),并手工检索相关杂志,纳入从2003年至2013年国内研究降糖药药物经济学的随机临床干预研究文献,并对其进行分析。结果:最终纳入文献22篇,单纯研究西药降糖药的有21篇,涉及中药的有1篇。研究均是实际临床试验,比较不同降糖药的成本效果。研究的主要成本指标是直接药物成本,效果是血糖的改善情况,研究人群均为无并发症的2型糖尿病患者。研究的周期最长48周,最短4周,平均16周,仅有1篇进行随访。11篇文献考虑了不良事件的发生。结论:目前国内降糖药的药物经济学研究主要存在:方法不统一、成本计算和效果方法衡量标准不一等问题,导致已有文献无法进行综合分析,不能得出最佳成本效果的药物。今后的研究需要探寻相应的标准和方法,使各研究具有可比性。
OBJECTIVE: To understand the domestic literature on drug economics research hypoglycemic agents, seeking cost-effective drug programs to explore the current domestic hypoglycemic cost-effective research deficiencies. METHODS: SinoMed, China Academic Journal Full-text Database (CNKI), Wanfang Database and VIP Chinese Science and Technology Database (VIP) were searched by computer. The related journals were searched by hand and included in the national research from 2003 to 2013 Literature on Randomized Clinical Interventions for Hypoglycemic Drug Economics and its Analysis. Results: Twenty-two articles were finally included in the study. There were 21 articles about western medicine hypoglycemic agents alone and 1 article on traditional Chinese medicine. All studies are actual clinical trials to compare the cost effectiveness of different hypoglycemic agents. The main cost indicator of the study is the direct cost of medication, the effect is the improvement of blood glucose, the study population are non-complication of type 2 diabetes. The study period of up to 48 weeks, the shortest 4 weeks, an average of 16 weeks, only 1 were followed up. Eleven papers considered the occurrence of adverse events. CONCLUSION: At present, there are mainly studies on pharmacoeconomics of hypoglycemic agents in China. The methods are not uniform, the cost calculation and the method of measurement of effects are different, leading to the fact that the literature can not be comprehensively analyzed and the best cost-effective drugs can not be obtained. Future studies need to explore the appropriate standards and methods to make studies comparable.
