目的研究并分析肿瘤化疗患者经外周静脉置入中心静脉导管(PICC)的消毒方式,以降低导管感染的发生率,提高临床护理质量。方法选取2011年6月—2014年5月收治的100例符合入选条件的肿瘤患者作为研究对象,随机分为试验组和对照组,各50例。试验组输液前正压接头的阴性端采用顺—逆时针两步消毒法,每次用复合碘消毒液棉棒螺旋式用力摩擦15 s;对照组采用同样消毒液棉棒顺时针螺旋式消毒正压接头的阴性端15 s共2次。两组患者分别于置管7 d和14 d进行PICC导管采样培养,14 d正压输液接头和PICC置管前端培养。两组结果比较采用χ2检验。结果试验组置管7 d、14 d PICC导管培养,14 d正压输液接头及PICC置管前端细菌学培养与对照组相比差异均有统计学意义(均P<0.05)。结论采用复合碘消毒液棉棒顺—逆时针两步消毒法可减少细菌从接头处侵入导管并定植,有效降低导管感染率。
Objective To study and analyze the disinfection methods of peripherally inserted central venous catheter (PICC) in patients with tumor chemotherapy to reduce the incidence of catheter infection and improve the quality of clinical nursing. Methods Totally 100 cancer patients eligible for inclusion in our hospital from June 2011 to May 2014 were selected as study subjects and randomly divided into trial group and control group with 50 cases each. The negative end of the positive pressure connector before the infusion in the test group was disinfected by a cis-counterclockwise two-step disinfection method, each rubbing force was rubbed with a spiral of iodine disinfectant liquid swab for 15 s, and the control group was disinfected with a clockwise spiral disinfection Crimp the negative terminal of 15s a total of 2 times. Two groups of patients were catheterized for PICC catheterization on day 7 and day 14, respectively, and catheterized on the 14-day positive pressure infusion catheter and PICC catheter. The two groups were compared using χ2 test. Results There were significant differences between the experimental group and the control group on the 7th day and the 14th day of PICC catheter culture, the 14th day positive pressure infusion connector and the PICC front end bacteriological culture group (all P <0.05). Conclusion The iodine disinfectant compound cotton swab cis-counterclockwise disinfection method can reduce bacterial invasion from the joint at the catheter and colonization, effectively reduce the catheter infection rate.
