目的 探讨上皮细胞黏附分子 (EP CAM)和 β 环连蛋白 (β catenin)在子宫颈鳞癌癌变中的可能作用。方法 用免疫组化染色的方法检测 14例正常宫颈鳞状上皮、32例宫颈上皮内瘤变(CIN)、38例宫颈浸润性鳞状细胞癌标本中EP CAM、β catenin蛋白的表达情况。 结果 EP CAM在正常宫颈鳞状上皮组、CINⅠ组、CINⅡ组、CINⅢ组和宫颈鳞癌组细胞膜的过表达率分别为 0、7.1%、2 0 .0 %、6 2 .5 %和 5 5 .3% ,其中CINⅢ组和宫颈鳞癌组EP CAM的过表达率均明显高于正常组和CINⅠ组 (P <0 .0 0 1)。正常宫颈鳞状上皮未见 β catenin的异常表达 ,CINⅠ、Ⅱ、Ⅲ和鳞状细胞癌组β catenin的异常表达率分别为 2 8.6 %、4 0 .0 %、6 2 .5 %和 84 .2 % ,β catenin异常表达率随CIN分级的提高和癌变而相应增高。EP CAM的过表达与子宫颈鳞癌分化程度呈负相关 (P <0 .0 0 1)。结论 EP CAM、β catenin可能参与了子宫颈鳞状上皮的恶性转化过程 ,EP CAM的过表达、β catenin的异常表达可作为子宫颈鳞状上皮癌变的参考依据。
Objective To investigate the possible role of epithelial cell adhesion molecule (EP CAM) and β catenin in carcinogenesis of cervical squamous cell carcinoma. Methods Immunohistochemical staining was used to detect the expression of EP CAM and β catenin in 14 cases of normal cervical squamous epithelium, 32 cases of cervical intraepithelial neoplasia (CIN) and 38 cases of invasive cervical squamous cell carcinoma. Results The overexpression rates of EP CAM in normal cervical squamous epithelium, CINⅠ, CINⅡ, CINⅢ and cervical squamous cell carcinoma were 0,7.1%, 20.0%, 62.5% and 55% .3%. The overexpression rates of EP CAM in CINⅢ group and cervical squamous cell carcinoma group were significantly higher than those in normal group and CINⅠ group (P <0.01). Abnormal expression of β catenin was found in normal cervical squamous epithelium. The abnormal expression rates of β catenin in CIN Ⅰ, Ⅱ, Ⅲ and squamous cell carcinoma were 28.6%, 40.0%, 62.5% and 84 respectively. 2%. The abnormal expression of β catenin increased with the increase of CIN grade and canceration. The overexpression of EP CAM was negatively correlated with the differentiation of cervical squamous cell carcinoma (P <0.01). Conclusion EP CAM and β catenin may participate in the malignant transformation of cervical squamous epithelium. Overexpression of EP CAM and aberrant expression of β catenin may serve as reference for cervical squamous cell carcinogenesis.
