目的探讨增强CT在口腔颌面颈部多间隙感染中的应用评价。方法 46例口腔颌面颈部多间隙感染患者作为观察对象,经患者口述和X线片初步扫描,医生确诊为口腔颌面颈部多间隙感染。为了更精准诊断病情病因,对每例患者都做了增强CT扫描。通过CT成像表现查明病因和受累间隙。结果牙源性感染32例;腺源性感染7例;颞下颌关节感染1例;颌骨外伤感染1例;发育性囊肿继发感染1例;其他各类继发感染共2例;只有2例未明病因。病因检出率95.7%。累及间隙有下颌下间隙27例,咬肌间隙8例,翼颌间隙3例,舌下间隙1例,咽旁、咽后间隙各2例,颊间隙和颈深间隙各1例,颞下间隙1例。感染检出率100%。结论通过本次观察,建议在口腔颌面颈部多间隙感染中的检查中首推增强CT。
Objective To evaluate the application of enhanced CT in multiple nosocomial infections of oral, maxillofacial and cervical regions. Methods Forty - six patients with oral, maxillofacial and cervical multiple space infections were selected as the observation subjects. After oral and X - ray films were scanned initially, the doctor diagnosed multiple oral and maxillofacial infections. In order to more accurately diagnose the etiology of the disease, an enhanced CT scan was performed on each patient. Identify the cause and affected space by CT imaging. Results There were 32 cases of odontogenic infection, 7 cases of gonococcal infection, 1 case of temporomandibular joint infection, 1 case of mandibular traumatic infection, 1 case of secondary cyst infection, 2 cases of other secondary infections, Cases of unknown etiology. Etiology detection rate of 95.7%. There were 27 cases of submandibular space involvement, 8 cases of masseter muscle space, 3 cases of wing-jaw space, 1 case of sublingual space, 2 cases of parapharyngeal and pharyngeal space, 1 case of buccal space and deep cervical space, 1 case. Infection detection rate of 100%. Conclusions Through this observation, it is suggested that CT should be emphasized in the examination of multiple space infections in oral and maxillofacial neck.
