目的 探讨超声在小儿睾丸肿瘤诊断中的应用价值。方法 对 12例小儿睾丸肿瘤应用 7 5~10 0MHz高频超声检查 ,并进行血清胎儿甲种球蛋白 (AFP)定性。均经手术、病理证实。结果 卵黄囊瘤 9例 ,AFP定性全部阳性 ;良性畸胎瘤 2例 ,恶性畸胎瘤 1例 ,此 3例AFP定性均阴性。超声图像共同特征 :睾丸增大 ,内呈不均质中强回声改变。卵黄囊瘤见不规则无回声暗区 ;畸胎瘤呈囊性多房改变或见液暗区外 ,有钙化强光斑伴声影。结论 超声检查小儿睾丸肿瘤 ,图像清晰、直观、无创无痛 ,小儿易于接受 ,具有较高的临床应用价值 ,可作为小儿睾丸肿瘤的首选影像检查手段。
Objective To investigate the value of ultrasound in the diagnosis of pediatric testicular tumors. Methods Seven hundred and fifty pediatric testicular tumors were examined by high frequency ultrasonography at 75-1000 MHz and serum fetal A globulin (AFP) was qualitatively analyzed. After surgery, pathology confirmed. Results Yolk sac tumor in 9 cases, AFP qualitative all positive; 2 cases of benign teratoma, malignant teratoma in 1 case, 3 cases of AFP qualitative negative. Common features of ultrasound images: Testicular enlargement, inhomogeneous hyperechoic changes. Yolk sac tumor see irregular anechoic dark zone; teratoma cystic sexual room change or see the dark area outside, there are calcified spots with sound shadow. Conclusion Ultrasound examination of children with testicular tumors, clear, intuitive, noninvasive and painless, pediatric easy to accept, with high clinical value, as pediatric testicular tumors of choice for imaging examination means.
