IMA、H-FABP及cTnI联合检测对早期急性心肌缺血的诊断价值

目的探讨血清中缺血修饰蛋白(IMA)、心型脂肪酸结合蛋白(H-FABP)及心肌肌钙蛋白I(cTnI)联合检测在急性心肌缺血进展中的诊断价值。方法将86例诊断为心肌缺血的患者和选取的48名健康人群分别检测IMA、H-FABP、cTnI水平,另外对心肌缺血患者组每隔2小时进行一次上述指标的检测并记录。统计三项指标单独及联合检测在心肌缺血进展诊断中的灵敏度及特异性。结果研究组患者的三种指标水平明显高于对照组(P<0.05);其中IMA灵敏度为89.5%,特异性为41.2%,H-FABP灵敏度为93.0%,特异性为86.7%,cTnI灵敏度为41.8%,特异性为95.1%。三项指标联合检测的灵敏度为96.5%,特异性为98%。结论三项心肌标志物联合检测对早期急性心肌缺血的诊断具有重要价值。 Objective To investigate the diagnostic value of combined detection of serum IMA, H-FABP and cTnI in the diagnosis of acute myocardial ischemia. Methods The serum levels of IMA, H-FABP and cTnI in 86 patients diagnosed as myocardial ischemia and 48 healthy people were measured respectively. The patients with myocardial ischemia were detected every 2 hours and recorded. Statistical sensitivity and specificity of three indicators alone and in combination in the diagnosis of myocardial ischemia. Results The sensitivity and specificity of three indicators of study group were significantly higher than those of the control group (P <0.05). The sensitivity and specificity of H-FABP were 89.5%, 41.2%, 93.0%, 86.7% respectively. The sensitivity of cTnI was 41.8% and specificity of 95.1%. The combined sensitivity of the three indicators was 96.5% with a specificity of 98%. Conclusion The combined detection of three myocardial markers has an important value in the diagnosis of early acute myocardial ischemia.
