目的探讨2型糖尿病患者听力损失的相关因素。方法对95例2型糖尿病患者(糖尿病组)及80例正常对照组进行纯音听力测试,同时对糖尿病组检测颈总动脉内径、颈动脉内膜中层厚度、颈部动脉粥样硬化斑块、空腹血胰岛素含量、纤维蛋白原、凝血酶原时间、血小板等7项可能的相关指标,分组比较及应用多自变量Lo-gistic回归分析探讨对听力的影响因素。结果糖尿病组听力异常者检出率为63.16%(60/95),对照组为40.0%(32/80);95例2型糖尿病患者中,听力正常组(35例)与听力减退组(60例)之间的颈总动脉内径、凝血酶原时间、血小板、颈动脉内膜中层厚度差异有统计学意义(P<0.05);42例颈部动脉无斑块者的语频及高频听力减退检出率分别为26.19%(11/42)、47.62%(20/42),而在53例颈部动脉有斑块的患者中分别为47.17%(25/53)、71.70%(38/53),均明显高于无斑块者(P<0.05),且高频听力减退更明显;Logistic回归分析显示,颈动脉内膜中层厚度、血小板、凝血酶原时间、空腹血胰岛素含量为2型糖尿病患者听力损失的相关因素。结论 2型糖尿病患者中有颈部动脉粥样硬化、高凝血倾向及高胰岛素水平者听力损失比例高。
Objective To investigate the related factors of hearing loss in type 2 diabetic patients. Methods Ninety-five patients with type 2 diabetes mellitus (DM) and 80 normal control subjects were tested for pure tone hearing. Diabetes mellitus was used to detect the diameter of common carotid artery, the intima-media thickness of carotid artery, the atherosclerotic plaques of neck, fasting Blood insulin content, fibrinogen, prothrombin time, platelet and other 7 possible related indicators, group comparison and application of multiple independent variables Lo-gistic regression analysis to explore the impact of hearing factors. Results There were 63.16% (60/95) cases of hearing loss in the diabetic group and 40.0% (32/80) in the control group.
