目的观察甲泼尼龙联合特布他林、异丙托溴铵治疗毛细支气管炎的临床效果。方法将2012年9月—2014年2月收治的100例毛细支气管炎患儿随机分为观察组和对照组各50例,对照组给予常规治疗,,观察组在对照组基础上给予甲泼尼龙琥珀酸钠注射液1~2 mg/(kg·次),加入5%葡糖糖注射液30~50 ml静脉滴注,1次/h,疗程3 d。特布他林2.5 mg,异丙托溴铵250μg溶入生理盐水2 ml,雾化吸入每1次/8 h,以氧气驱动(氧流量6~8 L/min)。比较两组咳嗽缓解时间、喘憋缓解时间、肺部啰音消失时间、住院时间及疗效。结果观察组咳嗽缓解时间、喘憋缓解时间、肺部啰音消失时间及住院时间分别为(4.25±1.28)、(3.17±0.90)、(5.98±1.65)、(7.05±2.15)d,与对照组的(5.45±1.18)、(4.80±0.95)、(7.40±1.70)、(8.60±2.14)d比较差异均有统计学意义(均P<0.05)。观察组总有效率94.0%,高于对照组的81.0%,两组比较差异有统计学意义(P<0.05)。结论静脉滴注甲泼尼龙联合雾化吸入特布他林、异丙托溴铵对于治疗毛细支气管炎安全有效,疗程缩短,值得推广。
Objective To observe the clinical effect of methylprednisolone combined with terbutaline and ipratropium bromide in the treatment of bronchiolitis. Methods 100 cases of bronchiolitis admitted from September 2012 to February 2014 were randomly divided into observation group (50 cases) and control group (50 cases). The control group was given routine treatment. The observation group was given methylprednisolone Sodium succinate injection 1 ~ 2 mg / (kg · times), add 5% glucose injection 30 ~ 50 ml intravenous infusion, 1 time / h, treatment 3 d. Terbutaline 2.5 mg and ipracopylard 250 μg were dissolved in 2 ml normal saline and inhaled once every 8 h with oxygen (oxygen flow 6 ~ 8 L / min). Cough relief time, asthma relief time, pulmonary rales disappearance time, hospital stay and efficacy were compared between the two groups. Results The cough relief time, asthmatic relief time, pulmonary rales disappearance time and hospital stay in the observation group were (4.25 ± 1.28), (3.17 ± 0.90), (5.98 ± 1.65) and (7.05 ± 2.15) days, respectively. Compared with the control (5.45 ± 1.18), (4.80 ± 0.95), (7.40 ± 1.70) and (8.60 ± 2.14) d respectively. There were significant differences between the two groups (all P <0.05). The total effective rate in the observation group was 94.0%, which was higher than that in the control group (81.0%). There was significant difference between the two groups (P <0.05). Conclusion Intravenous injection of methylprednisolone and inhalation of terbutaline combined with atomization, ipratropium bromide for the treatment of bronchiolitis safe and effective, short course of treatment, it is worth promoting.
