肺超声在血液透析患者容量超负荷评估中的应用

维持性血液透析患者大多存在不同程度的体液潴留,高容量负荷使心、脑血管疾病发病率和死亡率显著增加.目前血液透析患者容量评估主要采用临床评估法,即肾科医师根据经验、患者透析史和部分参数(如临床症状、体质量、血压、心率、水肿及血管充盈程度等)来判断[1-2],但主观性较强,不能有效评估容量负荷.因此,肾科医师一直在寻找一种敏感性高、特异性强、简便、非损伤、易随访且具预见性的方法来评估血液透析患者的容量状态,以实现患者容量负荷个体化管理.而肺超声可能符合上述特点,体现了其对维持性血液透析患者容量评估的特殊价值[2-3].
