目的为设计一种新术式(带血管桥的多根肋骨瓣)修复手掌毁损伤提供解剖学依据。方法在30侧经动脉灌注红色乳胶成人背部标本上,观察第7、8、9肋肋角外侧8cm段的形态结构及肋骨间血管、神经在该段的走行、分支、分布和吻合;在2侧尸体标本上模拟手术。结果该段的肋骨高[(1.4±0.2)cm,x±s,下同],肋间隙宽(1.0±0.3)cm,肋间动脉、静脉于该段的起始外径分别为(1.4±0.2)mm、(2.1±0.4)mm,终末支外径分别为(1.0±0.2)mm、(1.3±0.3)mm。结论设计带血管桥的多根肋骨瓣修复手掌毁损伤在解剖学上具有可行性。
Objective To provide an anatomical basis for the design of a new surgical procedure (multiple rib flap with vascular bridge) to repair hand-wound injuries. Methods: The shape and structure of the 8 cm segment outside the ribs 7, 8 and 9 and the movement, branching, distribution and anastomosis of the blood vessels and nerves in this segment were observed on the 30 dorsal arterial instillation of the red latex. Side of the body specimens simulated surgery. Results The rib height [(1.4 ± 0.2) cm, x ± s, the same below] and the intercostal space width (1.0 ± 0.3) cm in this segment of the intercostal artery and vein were (1.4 ± 0.2 mm and 2.1 ± 0.4 mm respectively. The diameters of the terminal branches were (1.0 ± 0.2) mm and (1.3 ± 0.3) mm, respectively. Conclusion It is anatomically feasible to design a multi-ribbed flap with a vascular bridge to repair the damaged hand of the hand.
